RE: CK

CK was examined on January 9,2001. At the time of evaluation, the primary concern was regarding CK’s tendency to close one eye when he points to a particular object which has been occurring less frequently lately. CK also demonstrates unusual visual behaviors including opening his eyes very wide, crossing his eyes, squinting and turning his head to point. These behaviors were first noted at approximately one year of age.

CK’s general health is reportedly good. At the time of evaluation he was taking no medications, had no reported allergies and was taking vitamin supplements. CK has been receiving educational, speech, and occupational therapy through Early Intervention Services.

Uncorrected visual acuities measured 20/40 right eye and left eye at distance; 20/30 right eye and left eye at near with Lea contour interaction charts. Fixation was steady and accurate with no indication of nystagmus. Localization was also accurate with both the right and left eye. Muscle fields were full without restriction and visual tracking was demonstrated into all positions of gaze. Near point of convergence was satisfactory. Tests of sensory and motor fusion indicated stable binocularity (eye teaming) with no indication of suppression. Refractive findings indicated low hyperopia which is entirely age appropriate. CK also demonstrated stereo sensitivity to 100 seconds of arc.

All external eye health findings were normal as were pupillary responses and confrontation visual fields. Funduscopic examination indicated healthy eye grounds in each eye with no apparent pathology.

Based on these findings, CK demonstrates healthy, normal eyes and satisfactory vision development. Mild hyperopia is expected at this age and requires no lens treatment. The behaviors CK has been demonstrating are most likely related to his underlying autism rather than to a visual limitation.

Thank you for the opportunity to participate in CK’s care. Should you have nay questions or concerns, please feel free to contact me.
